This is a great application form. I would like to see a little modification to allow room for the last 5 years worth of previous addresses. Thank you for making such a great tool available!

I am new to real estate investing and am converting my former home to a rental. I found this really helpful. like the clean layout. It is easy to fill out and read. And since it is in Word format I was able to make a few adjustments on my own - I hope you don't mind. I am happy to share my version if you like.

In the below the lines you start with have you been convicted of. At the end you have in parentheses "weather or not it resulted in a conviction" Would it not be better to start it with "Have you ever been arrested, plead guilty, or no contest to any charge involving sexual misconduct or a crime against a child" Just a thought.
