Sample offer to purchase

For all the wholesalers/flippers out there, what do your offer to purchase forms look like? I'll be looking to flip the contract or the house so I'm curious as to what kind of wording is in there. Also, is there anything you guys include that gives you an out if either you can't assign it or come across a more profitable flip? Looking forward to your responses.
