Writing an offer for Seller Financing

I am interested in purchasing a home as an investment property. (The home is located in NY.) I want to offer 10% down with seller financing. Assuming the seller owns the property free and clear, how do I structure an offer for seller financing? The terms aren't in the pre-filled contract to buy, so how do I specify the terms?
