The few times i have looked at a house to estimate repairs, i used a mix of my move in condition form i give my tenants, and the inspection form i got when i got my house inspected.

Just get a real contractor's estimate written on their letterhead that they use. If you produce your own, don't you think this could get discovered?
Now, if you are looking for a form you can use to get a ballpark figure before you make offers, that's a different story - several BP members will send you theirs if you PM them.
I'm not sure if it will meet your needs, but I have used insurance adjusting software to do a loss/damage estimate to a property after a tenant moved out. I used Xactware, but I'm sure there are others out there.
