Closing Cost? Cash at Closing?

Hey BP family.

I am closing on my first property at 211,000, quadplex with a FHA at 3.5%. I am not too sure about closing cost details. But does anything look off. I thought my closing cost is very high, $12,000.

Also, my downpayment says $3,822. Shouldn't it be like 3.5% or about $8,000? 

AM i reading this wrong?
