Need help filling out rental application section 8

On the last page of rental application for section 8 their is a 3 blank section to fill out for putting.

They look like this:

Before that it says to Print the tenant name and Tenant's signature & Date which I already did for myself. But, What do I put their? PLEASE HELP ME as I need to do this ASAP!
