Dams can be caused by a number of issues, not limited to, but including poor process adherence (personnel issue), misaligned processes (process drift), or process friction (environment, timing, etc.).The source of a dam isn't always immediately obvious.

I also do not subscribe to the absolute "no visitors" policy - but I have seen it where the locality implemented strict "no visitor" policies and make you adhere to your max occupancy.
