Forfeiture Notice

I have someone I need to have removed from a home we have it is a land contract deal.

I am looking for a Forfeiture Notice in order to get the process started.

Additionally, any other information would be help full.

I am sure I can have them removed by standard eviction proceedings as they are under 20% paid on the property, but I do need to find a Forfeiture Notice.
