This is a very basic form, all employees and suppliers MUST sign in when working/delivering materials. YOU NEED THIS ON SITE AND FILLED EVERYDAY! ** THIS WILL PREVENT LIENS!! ** By letting everyone sign this, GC, subcontractor on all tiers, suppliers, and etc, you will know who and what was used/performed service for your property. Make sure on your contract, you will require your contractor who signs the contract to fill this up without fail. If they used a subcontractor, they will need to sign also, then, during payments, you will need to collect this, have your contractor sign every page and "Certify" all pages that it is all correct and complete, plus an Unconditional waiver and release from ALL subcontractors/suppliers and the conditional waiver and release from the contractor.
