Pennsylvania Landlord-Tenant Complaint Form Help Request

Please help me out! I need to evict a tenant for non-payment (my first eviction). 

I live out-of-state and so I'm unable to visit the magistrate's office for assistance with filling out the paperwork. 

I know hiring a lawyer is an option but I would like to mail the landlord-tenant form and filing costs in and then fly in once a date has been set.

I've already had the 5-day notice posted. I downloaded the landlord-tenant complaint here: http://www.pacourts.us/assets/files/setting-901/fi...

but there were a few sections I was unsure as how to fill out. I looked both online and on BP for an example of a filled-out landlord-tenant form but I couldn't find one and so here I am hoping for a fellow landlording Pennsylvania BP member who has filled this form out previously to post an example of a filled-out PA landlord-tenant form. 

Even if the form is filled out with a made-up name and address that's fine - it would still be amazingly helpful to me. I simply hope for a filled-out template I can refer to. 

I appreciate it so, so much. Having this help will mean a lot and be very much a help. Thanks.
