Pennsylvania Landlord-Tenant Complaint Form Help Request

Please help me out! I need to evict a tenant for non-payment (my first eviction). 

I live out-of-state and so I'm unable to visit the magistrate's office for assistance with filling out the paperwork. 

I know hiring a lawyer is an option but I would like to mail the landlord-tenant form and filing costs in and then fly in once a date has been set.

I've already had the 5-day notice posted. I downloaded the landlord-tenant complaint here: http://www.pacourts.us/assets/files/setting-901/fi...

but there were a few sections I was unsure as how to fill out. I looked both online and on BP for an example of a filled-out landlord-tenant form but I couldn't find one and so here I am hoping for a fellow landlording Pennsylvania BP member who has filled this form out previously to post an example of a filled-out PA landlord-tenant form. 

Even if the form is filled out with a made-up name and address that's fine - it would still be amazingly helpful to me. I simply hope for a filled-out template I can refer to.

    Kevin was nice enough to help me with the paperwork. 

    I haven't evicted that particular tenant yet (she had paid up but I'm not renewing her lease which is up November 30th) though I did evict the upstairs tenant. I was in town for that court date and it was simple - it lasted about 5 minutes and I got my judgement. 

    The nice thing is a high school friend is a lawyer and he said he'd let some of the younger lawyers in his firm take my future eviction hearing cases at no charge for the experience (all I need to do is pay the court fees and supply them with the paperwork/evidence).

    Now that was nice.  If you have the house paid off consider moving it to trust.  Help to protect your property from liens.  

    Did you gents hear about the lawsuit that was won in Ohio Supreme Court ruling rental inspections unconstitutional.   A win for landlords everywhere.  Especially in towns like Reading where the City Tyrants love to take your private property and apply it to public use without your consent ... unconstitutional.   They are also guilty of ijfringing on interstste commerce with their "business privilege tax".  There is a UCC code that prevents that.  Legal and lawful are very different.   We need to open our eyes and keep the money in our pockets.

    I’m in a similar situation as Karl. I have to file my first for non payment of rent.  I was hoping someone could share their form as well. Thanks!!

    Where in Pa is your property?

    Going forward, wouldn’t it be better to have a PM instead of flying out for a 10 minute hearing?

    NWPA. And nope. I was in town for a high school and family reunion so I didn't need to fly in to file the paperwork nor attend the hearing. Also, PMs in my area are garbage. 

    I had a second eviction recently and as mentioned, my friend (lawyer) took care of it without charging me.

    Alright. Here's one, altered to lighten the mood. For the upper right corner you'll easily be able to locate the phone number and address to the correct local magistrate. Call them to get the correct District Number. 

    You don't fill in the filing costs, constable ed., etc section - they'll do that after the hearing. 

    As one can see, this form was filled out for non-payment. 

    When I needed helped with the form a BP member told me not to fill out the line marked total. I took his advice. If they want that line filled out simply hand write the total when you're at the magistrate's office.

    It's not a complaint I want to sue my landlord but I looked in the forms and I can't find mdjs 30 ba-bl form anywhere
