Hello Kristina, I use turbo tenant and it has landlord forms for every state built in, it also allows an area in where you can insert your own tenant rules (addendum).

Its a half folded card stock insert that is white on the inside.  

They have 1 week (insert date) to let you know if they are renewing the lease, otherwise you are giving them notice of non-renewal.
