Certification of trust

I’m wondering if anyone here has prepared their own certification of trust. I have the county assessor wanting a Certification of trust form filled out due to one of my properties being titled in the name of my trust. in order to avoid reassessment.

I’m wondering if I really need an attorney To prepare this for me, since it appears to be such a simple form to fill out, and avoid an attorney fee. Any advice welcome.
