Handyman retainer agreement?

Hi, does anyone have a sample handyman retainer agreement that they would be willing to share? This would be for a weekly retainer – someone who would keep an eye on the property, respond to maintenance requests, and coordinate with other contractors. He is a licensed plumber and heating contractor as well. Many thanks!
