How to make an offer without agent?

I have only ever used an agent. So if I wanted to make a formal offer without an agent, what do I need to do?

What documents do I use? Where can I get those documents? What do I fill out ahead of time/with the seller?

Do I just email/mail/deliver a document with my offer and a cover letter?

@Andy Thoman If you're so new that you're asking about which documents to use, I'd suggest that using a buyer's broker is still the best course of action.

It's not just about forms.  It's about negotiating experience, local knowledge, understanding what the seller is required to disclose and what they will only disclose when asked, knowing how and when to chase paperwork for signatures and extensions, etc.

Real estate is like pro baseball.  It looks easy from the bleachers, but when there's a 90 MPH fastball coming at your noggin, you realize that that this is a place for experts.

No doubt that with enough experience you'll be able to go it alone, but it doesn't sound like you're there yet.
