Transferring a Property to an LLC

If I purchase a multi-family in my own name, with a mortgage, how can I transfer that property to an LLC? If I transfer the property to an LLC, will my mortgage be called due? How can I avoid that from happening?

I reside in NY, so I am assuming I will need to set up the company in NY? How does an LLC / Trust work the most advantageously in unison?
