Rental Application (pre-lease form)

Hi Everyone.

I'm new to the LL business.
I finally got my units clean and ready to rent.
I have some lease forms to work off of, and a good realtor friend helping me with most things.

Was wondering if anyone has, or can point me to a link, where I can find an already formatted (.doc/.txt/etc) file of an application.

I found one online when I first got the building, but for the life of me can not find it or something similar now...

Just looking for something basic for screening, with name. addy ss#, consent for backgroundcheck/credit check, prior landlords, employer info, references, type info form that I can print out.
