Three day notice to quit in CA

Hi all,. First rental property, first tenant and now sending a three day notice to pay or quit four months into the lease. I admit I have been lenient with my tenant and have now allowed him to be two months behind(February and now March). A few questions before I fire the notice off. the property is in the LA City area if that matters.  Nothing like on the job training. 

1. Should I send via Certified mail with receipt on delivery

2. The lease I am using allows the tenant to pay late fees up to 20 days. When do I send the letter after the 1st of the month when rent is due or after the 20 day late period? 

3. Just want to confirm that the Three day notice to pay or quit is the correct form for CA for nonpayment of rent. 

4. Do I need to send a separate notice for each month or can I combine the two months and write that this amount is representing the period between 2/1-3/31.
