Commercial Property Grounds Management

I have a 5 unit medical complex and one of the tenants provides urine tests for the state. As you can imagine, the unit has high traffic of patients and some  problems have come up:

1. Trash is being deposited behind the dumpster, rather than in it. This is a direct cause from the staff.

2. The patients and their accompanying friends are leaving the parking lot scattered with trash on a constant basis.

3. The patients have been lining outside of the office entrance, blocking the entrance to the other units.

This of course is creating a problem for the other tenants and their patients, even thought the problem tenant has been notified on two separate occasions. We have thought of installing a guard rail in order to guide their patients away from the entrances of the other tenants. However, keeping the grounds clean is another issue. Can fines be assessed, even though it's not in the lease? 

Can someone provide some thoughts on how to best tackle this situation?
