Rehab Walk Through Form

Do any of you guys use a paper form to help on your walk through of a property? I have spreadsheets and things to calculate repair costs. What I am after is a form that lets you go from room to room noting dimensions and defects. I'm thinking something simple like a square for each room with some spaces for dimensions and an area for notes off to the side. The square would represent the room and I could note location and size of any defects or things like counter tops, sinks, toilets, light fixtures etc. 

Any thoughts?

Some of the flippers not only have a form or spreadsheet but they do all their homes with the same basic materials so they also have the home depot part number for each item right on their form.  When they walk a room they note if it needs a light fixture, paint, closet rods, sink, toilet, shower tile, door, door knob etc.  When they walk out they are ready to go to the Home Depot Bid desk and place their order for delivery (now they have a materials price).  Very efficient in many ways.
