Walk thru Rehab estimated cost sheet

I have been reading a lot of these posts and If I remember correctly there was a walk thru rehab  cost sheet relating to all the items that had to be done or removed so you could figure a more accurate rehab cost. 

Is it here if not can you direct me towards it?
