I need an Independent Contractor Agreement form

I have done few rehabs with General Contractor who i knew personally but now am plan on doing rehabs in different market  deal with GC i don't know  so i was wonder if anyone  can share with me  an Independent Contractor Agreement form, along with a Lien Waiver form. thanks
