Criticize my landing page, please

Hi folks,

Hope this is the correct place to open this kind of post.

I have been using FB ads for a few weeks now and my main problem consist in a very low conversion in my landing page. I have tried a couple of approaches but can't seem to nail the correct message.

I based my design and message on a sample postcard I posted here in BP that had good success as per the user.

Below is the landing page screenshot. Would you mind to criticize it and give me some hits as of why it is not converting well?
