60 day notice to vacate template - Chicago

Hello,

Looking to see if anyone has a template on sending a notice to tenant to vacate the property in 60 days. Currently tenant is MTM lease in Chicago and has resided in the property less than 3 years.

If no template exist, what information is good to add on the notice?

I’ve seen a lot of Illinois templates but looking for chicago specific.
