NYS-Compliant Eviction Notice (First Step)

I'm in the unfortunate position of needing to evict a non-paying tenant after three months of false promises and lies about new jobs. Does anyone have a New York State compliant first-step eviction notice that I can serve on the tenant? Newburgh City and New York State are extremely strict and the law seems to keep changing. Thank you for your help in saving hundreds of dollars in legal fees for the first step in the process!
