Lead based paint disclosure forms for wholesale deals?

I recently heard a few people in my investor group talking about the necessity of having seller sign a lead based paint disclosure form when they sign the contract.  

Does anyone have  form we could use for the state of Indiana?  The one I found asks for "broker's initials" in it, so I'm not 100% if I should use it.

    Leave the broker initials blank if the isn't one
