Option To Purchase Contract

OK, so I plan on using a Option To Purchase Contract for my next deal. I understand how to fill it out but what i do not understand is what do i sign with end buyer? 

how would i assign my option to purchase? 

Do I sign my end buyers Purchase and Sale Agreement?

I'm confused..
