Which form to use for leasing property to LLC?

We have a rental property with a loan so cannot place the property into our LLC for 12 months (per bank rules). We were advised by an attorney and CPA to instead lease our property to our LLC and then have the LLC lease to tenants in the meantime. Does anyone know which TREC form we would need to set this up? I searched the website but was not able to find an answer.
