VOIP Number/Provider for Business

I have a few VOIP numbers at this point through a basic provider. I can use them via my cell phone, computer, or VOIP phone at home. Usually, I just forward the calls to my cell to improve call quality, but then I never know what line was actually called when my phone rings.

What provider(s) do you use? What is their cost? Can you make new numbers on the fly? Why did you choose them?

@Bryan O. I use Google Voice.  It works well and is free.
