Agent Lender Relationship

Do you always need to go through both an agent and lender when purchasing a home? Or can I reach out directly to the lender?

You can start with a Lender.  I think one of the reasons why people like to start with the agent is because they may have many transactional partners that they know and have long standing relationships with.  This makes the transaction smooth when there are working relationships.  Also, the agent has already vetted them for you, so you don't have to worry about a Lender who doesn't give you the kind of customer service that you deserve.  With a referred Lender, they know if they mess up your loan, the agent will not be pleased, and likely stop using them.  So, it works in your favor.  However, you are free to select any vendor you wish to work with in your transaction.
