Lease extension form

Hi all, Where can I get a simple lease renewal form that says lease remain the same as original one and extend for one more year .

This is typically done with an Amendment... a new set of terms that gets agreed to, signed and then attached to the original lease. 

Let's say Section 2 of the original lease concerns the "term" of the lease being January 1, 2017 - December 31, 2017. An amendment to such a lease, if that is truly the only change, would be in writing and provide a new Section 2 stating that the term is January 1, 2018 - December 31, 2018; and it would further state that all other terms and conditions of the lease remain the same.
