Declaration to Affix

I'm developing a plot of land and putting a double wide on it. I went to a lawyer to get a Declaration to Affix but he's going to charge me $700. I was wondering if anybody has a blank i could use, or for North Carolina as far as I understand it it's a pretty standard form but I was wondering if anybody on here knows more about this than I do.
