Solo 401k Roth Conversion

Hello! I have a solo 401k and I have converted some of the traditional funds to Roth funds. The plan allows for it and I have all of that documentation, but what document is provided to the CPA so that the conversion can be reported to the IRS?

Bryan,

Plan administrator needs to issue a 1099-R to the plan participant and report the distribution to the IRS. We do that for our clients and then file electronically with the IRS. The bottom line you need properly completed IRS Form 1099-R.
