1st Owner = Individual 2nd Owner = Trust (CA). Problem?

Was a trust recorded incorrectly if it was filed as 2nd owner, leaving the original individual owner as 1st owner?

This is a personal matter where the original owner's new wife is not to be trusted.  Yet, she is the one that had the trust written and recorded.  The original owner is in his late 70's and his memory & cognitive function is on the decline.  She has lied and been very "crafty" with matters in the past and we just don't feel good about how controlling she is.

It's a revocable trust.

When he passes, can it be processed the same as if it were a property with a trust recorded as the only owner?  Or, will this create a problem for the beneficiaries of the trust?
