Where do most outbound systems actually break down?

I’ve been looking at a lot of different outbound setups lately—from fully manual teams to AI-driven systems—and it’s interesting how most of them are well built in certain areas but still break down in similar places.

Some are strong on:

  • list generation
  • automation
  • follow-up systems

But even with all that in place, there’s still a lot of inefficiency in the early stage—mainly around who actually gets engaged first.

It seems like most of the focus goes into building the list and executing outreach, but not as much into prioritizing which leads are most likely to actually connect and move forward.

Curious for those running more advanced setups—

Where have you seen the biggest bottlenecks in your process?
