Help filling out an assignment contract

Would anyone be able to show me or explain to me how to fill out an assignment contract for a wholesale deal? I live in Northern New Jersey and am talking to a few people about their properties but before I get any further I want to know if I come up with a deal, that I get the contract done properly.

it should be self explanatory. 

Why not wait till you get there. When you find a deal then start to ask how to figure it out.
