Does anyone have a fillable change of ownership form?

Does anyone have a change of ownership fillable form?  I just purchased a rental in Indianapolis and need a form to send to my new renters   BP landlord forms doesn’t seem to have one.   Any help would be appreciated!

LOL @Dennis Wayne that was exactly what I was looking for!    Ill just copy and paste that.

Thank you everyone who gave input.  I created my own doc and it turned out great.   It helps to see what others have done.  I dont want to look like a clown right out of the box.   They will discover that in time Im sure.....
