I'd also suggest reading some of Mike Butler's books, he's a former police detective out of Louisville that did pretty much the exact same thing you want to do.

You can then either build the site yourself (Weebly makes it easy) or post it as a job on a site such as Odesk or Elance.I highly recommend reading Landlording on Auto-Pilot by Mike Butler, there are some good ideas in there, too, about what to put in your forms.
