Here's an example of my log, I exported it from the REPSLog app:

I recommend logging your hours with an app called REPSLog and exporting it at the end of the year.
Personally I found it easier than using a spreadsheet. It's also an easy way to keep all supporting documents in one place.

I have been using the REPSLog App to log my hours and it's been way more convenient than a spreadsheet.
