Post: Multiple Residents on One Lease

My condo is in Utah. I would like to rent per room, but to follow the HOA, I would have to have one lease agreement. How do I put four individuals on one lease? Do I have one overall price at the top of the lease and have the individual price in the body of the lease agreement for them to choose from?
