I have a Google form that I send all prospective renters that ask for a viewing. Any feedback would be appreciated. By the way, I'm located in Massachusetts.The questions are:

*How long have you lived there?

Why are you moving?

Employer's Name 

Employer's Address (#street,city,state,zipcode) 

Your job title

How long have you worked there 

Monthly income *

How many people will be living with you?

Do you have any pets ? 

Do you, or anyone that will be living with you, smoke or vape?  

Month ($1,650), Last Month ($1,650), and security deposit($1,000) is required. Total $4,300. Do you have proof of funds ? 

What is your credit score *600 or below, 600-700, 700 and above

When is the best time to contact you to schedule a viewing
