You would use your LLC name and LLC checked is correct and use your EIN #. That is how I have mine for my properties I have in an LLC. I am a property manager and would advise my clients to do the same. If in doubt ask your accountant and they can answer based on your case.

Where do you use your LLC name: in Line 1 or Line 2? do you write the name of holding/parent LLC or subsidiary/operating LLC.

If anyone who has filled out a W-9 for property management please help me out.
I have formed a holding/parent LLC company (with me and my spouse as 50/50 members) and a subsidiary/operating LLC company which is 100% owned by the holding/parent LLC
I am filling out a W9 form for a property management company for a property I am closing under my
subsidiary/operating LLC
On Line 1: Do I write My Name?
On Line 2: Business Name/disregarded entity name: Write the name of subsidiary/operating LLC
On Line 3: I have checked Individual/sole proprietor or single-member LLC. Is that correct?
On Part 1: For TIN, do I enter EIN of subsidiary/operating LLC or my SSN?

Appreciate your help.
