That's what I initially thought would happen.  We'd come to end of the lease term,  I would give my notice (plenty of time), and then the co-tenant would get a new lease with the landlord.  I found out that the co-tenant may not qualify on their own and they asked me to just let them go month to month.  The landlord then said to me as long as someone is in "possession" of the unit, I would still be liable.

I ask if this person does not qualify for their own lease, what coarse of action to I have to get my name off the lease.  Basically, the landlord told me, not much, as we're both on the lease (that then goes month to month if we do not "move").  I asked, "How long would I be held hostage?  If the co-tenant decided to live there forever (my friend's parents have lived in their unit over over 20 years), I would liable until the co-tenant gets lease in their name or moves?  The landlord said, "Yes."

Doesn't seem fair.  I know the law isn't always fair, but this seems a bit extreme.

Lease is coming to term end and landlord said unless BOTH co-tenants move out that I will be liable for the unit.  I asked the landlord how I can go about getting off the lease, and he gave a couple of conflicting options.  I'm confused about it all.  The co-tenant and I are NOT on good terms and I don't think I can negotiate anything with them help with this transition.  Am I obligated to being hostage of this apartment unit?  How long?  If they stay there 10 years, am I also responsible until they leave?
