First Time House Hacking Addendum and Amendment INDIANA

Hi there I recently bought a home located in Indiana and I downloaded the biggerpockets lease agreement, but I'm having trouble understanding the residential lease addendum and amendment. Am I suppose to use both when I accept a new tenant? Or am I only suppose to use one? I'd like to house hack but I can't really step forward until I get this sorted out.

What about the "residential lease agreement" ?

(A) As used in this Lease, “Landlord” means
[_________________________________].
(B) As used in this Lease, “Tenant” means
[______________________________________]. The individuals comprising Tenant and the
children of such individuals, as set forth below, shall be the only occupants of the Premises. As
of the date of this Lease, the children of the individuals comprising Tenant are
[________________________________________________________]. Further, Tenant
warrants that Tenant is over the age of 18 and legally capable of entering into a binding
agreement.

What do I fill out in here?

I greatly appreciate any help.
