buying land contract in Ohio - how to transfer to our name

We are about to buy a Land Contract for a note & house in Ohio. We have seen a copy of the Purchase Money Note, Land Contract, & filed Warranty Deed. What do we need to transfer this to our LLC? I would assume an assignment & allonge but this is our first land contract purchase. Thanks.
