Rent Increase notice for a tenant who changed her last name

Hi everyone.

A tenant changed her last name after the lease was signed. I have a copy of her name change certificate. How should I write her name in the annual Rent Increase notice?

Is it a good idea to put the tenant's former last name in brackets, e.g. FirstName LastName (FormerLastName) ?
