Non-renewal of lease, need some language

Does any one have a sample non-renewal form they'd be willing to share? I looked in the files but no luck...

    You need a form for a tenant to say they are not going to renew???

    Do a quit claim of tenancy.

    Write a letter and have them sign it, we are packing our wigwam and moving on!

    We always just let them move out, do an inspection and go from there.....?? :)
