Los Angeles RSO - Statement # to register property? (APN)

I'm trying to register my condo with the City of Los Angeles (HCIDLA) so that I can receive a copy of my property's registration statement. I understand that I must pay an annual registration fee and submit my property to the LAHD Rent Registry. Regardless of whether I try to add my property to the billing site - housingbill(dot)lacity(dot)org/billing - or the rent registry website - registerlarent(dot)org - I'm asked for both my APN # and a statement #. I have my apn number but I have no idea what or where to find my statement #. My understanding is the statement # is comes on an annual bill maybe but I don't believe I've ever received that annual bill. This has me stuck and I have no idea how to move forward. Both sites say "If you do not have statement # for this property yet, please us the change of ownership application." but I can't find a change of ownership application either on either site.
