Posting and Filling Out a Notice to Quit

I am in Pennsylvania. I unfortunately have to start the eviction process. I'm going to fill out the Notice to Quit and post it. I had a couple questions regarding it. On the Notice to Quit, for past due rent amount should I include all of September's rent. I ask because rent is always paid ahead of time on the 1st. Or do I prorate the amount based on when I post it? My second question is on the Notice it says "Time Period" for the past due rent amount. Since this tenant was given many months to catch up the past due amount is not going to equate to the time period so what should I put there?

And if you have any other things you feel I should know please let me know. Thank you!

You rent by the month not by the day.  You should have an eviction attorney do this for you. Call them and get some prices. If you mess up one little thing you will have to start all over.
