breaking of lease

I have a tentant that can no longer afford the rent. I am happy to let her break her lease as long as i can find another suitable tentant. I told her verbally that i would get back to her on this. Shes been a annoyance thus far, smell of smoke coming from apartment so we are happy to break her lease. I told her verbally that shes liable for the lease until i find another suitable tentant. Is there a form i should have her sign or any advice would be great appreciated. Thanks in advance.
