Online Rental Application

I already use mysmartmove for credit/background checks, but if I want additional info like past landlords etc, I still need another application of some sort. Does anyone know of a good online platform that I can specify the additional info I need for free? I am sick of paper!

Cozy has one but I don't think they allow you to modify it, which sucks as I like to put in own questions. 

If you want to use your own app, I believe this can be done using google forms.  There's a thread here where a guy had some pretty extensive stuff done via google forms.
